Metformin, Vitamin B12 and Neuropathy: The Test Worth Asking For
If you take metformin and have tingling feet, one inexpensive blood test may matter more than any supplement purchase.
The link
Metformin is the most prescribed medicine for type 2 diabetes. In a randomized trial lasting more than four years, people on metformin had lower B12 levels than those on placebo, and more of them became deficient[1]. Low B12 damages peripheral nerves, producing numbness and tingling that look a lot like diabetic neuropathy.
Who is at risk
- Long-term metformin users, especially at higher doses.
- Adults over 60.
- People taking acid-reducing medicines long term.
- Vegans and strict vegetarians.
- People who have had stomach or bowel surgery.
What to ask for
Ask your doctor about a serum B12 level and, if it is borderline, methylmalonic acid. Neuropathy work-ups commonly include B12 alongside glucose testing[2]. ADA guidance recommends periodic B12 checks in people on long-term metformin, especially with neuropathy[3][4].
How B12-related nerve problems can differ
B12 deficiency can affect both peripheral nerves and the spinal cord, so in addition to numbness and tingling in the feet it may cause unsteadiness, especially in the dark, and changes in vibration sense. It can also cause anemia with large red cells, fatigue, a sore tongue and changes in mood or memory. Because these overlap with diabetic neuropathy and with ordinary aging, deficiency is easy to miss unless someone tests for it. Treated early, deficiency neuropathy can improve; long-standing damage may not fully recover, which is why testing matters.
Where supplements fit
If you are deficient, your doctor will recommend B12 tablets or injections at appropriate doses. Nervecept contains no B12, despite B12 imagery on its sales page, so it cannot correct a deficiency. Get the test first; then decide whether any other supplement is worth adding.
